Complete your clearances and endoscopy closer to home in Central Florida, then travel to South Miami for surgery with Dr. Reza Keshavarzi, who plans, performs and follows every case himself.
Before
AfterThese are Dr. K's own patients, photographed before surgery and again during their follow-up year. How much weight comes off depends on the procedure, the starting point and the habits each patient builds afterward.
Bariatric surgery1 of 6
Bariatric surgery reduces the size of the stomach and quiets the hunger hormones that usually win after a diet ends.
Dr. Keshavarzi performs these operations laparoscopically, through a few small incisions guided by a camera. With a gastric sleeve, the procedure most of our patients choose, he removes most of the stomach along its outer edge and leaves a slim tube that fills quickly. With a gastric bypass, he forms a small pouch and connects it farther along the small intestine, which also changes how food is absorbed.
For a patient in Four Corners, the surgery is the same whether you live five minutes or three hours from us. What changes is the planning around it, and that planning is built into every step below.

Driving several hours for a consultation should mean hearing about every option. Dr. K performs surgical, endoscopic and medical treatment himself, so his recommendation is not limited to one operation.
| Option | What it is | Worth knowing |
|---|---|---|
| Gastric sleeve | Laparoscopic removal of most of the stomach, leaving a narrow sleeve. | Offered as a single package price that covers the hospital and a year of follow-up. |
| Low BMI Mini Sleeve | A sleeve procedure planned for patients with less weight to lose. | Dr. K reviews whether your weight and health history fit it. |
| Gastric bypass | A small stomach pouch joined farther down the small intestine. | Dr. K advises it when your health history points to it. |
| Endoscopic sleeve gastroplasty | The stomach is stitched narrower from inside with an endoscope. | No external incisions. |
| ORBERA gastric balloon | A soft balloon placed in the stomach so you feel full sooner. | Temporary and nonsurgical. It is removed later, so plan for a second visit. |
| Medical weight loss | A personalized plan that can include GLP-1 medication and coaching. | A starting point, or support after a procedure. |
At your consultation, Dr. K explains which option, or which combination, suits your health and goals.
Four Corners sits where Lake, Orange, Osceola and Polk counties meet, so "near home" can mean a lab or endoscopy center in any of them. The trip south is kept for the steps that need Dr. K.
Dr. K meets you himself, reviews your weight history and health, and recommends a procedure. You leave with a plan and a price.
Medical clearances and an upper endoscopy are required. Have them near home, or with our recommended doctor in Miami, who returns results within 48 hours. An endoscopy from the last five months is accepted.
You follow a pre-surgery diet, and weight loss injections stop at least two weeks before the operation. Documents arrive by text, and you can reply the same way with questions.
Dr. K performs your procedure. How soon you are scheduled depends on how quickly your testing is complete.
Most patients are back at work within 4 to 7 days. Lifting over 20 pounds waits until you have healed.
Your follow-up visits for the first year come with the sleeve package, and Dr. K remains reachable by phone between them.

Dr. Reza Keshavarzi, MD, FACS, FASMBS
Patients know him as Dr. K. He is our lead surgeon, he holds every consultation himself, and he performs the operation he recommends, so no one hands your case to someone you have not met.
Distance is easier when your surgeon answers. Dr. K calls patients from his own cell phone and checks on them once they are home, which matters when home is a few hours north.
Dr. Keshavarzi was with us throughout the whole process and was even contacting us once home to see if everything was running smoothly.

Always available (even provides patients with his personal cellphone number) and takes his time to explain everything.

Mari was also a lot of help, she explained the diet thoroughly and was always happy to help.

Planning a trip for surgery is simpler around one known number. The gastric sleeve has a published package price, and Dr. K gives pricing for the bypass, ESG, balloon and medical weight loss at your consultation.
If Dr. Keshavarzi performs your pre-surgery endoscopy, the procedure itself is part of your overall cost and a $750 hospital fee applies.
Debit and credit cards are accepted for any remaining balance. Full terms are on our financing page.

The operation changes how much you can eat. What you choose to eat, how often you move and whether you keep your check-ins decide how much weight stays off, and Dr. K can add GLP-1 medication later if progress stalls.
These services keep that support going from the first weeks at home through the years after.
Yes. Medical clearances and the required upper endoscopy can be done at a facility closer to home, so most of your preparation happens in Central Florida. You travel to our South Miami office for the visits and the operation that need Dr. K in person, and our team stays in touch by phone and text in between.
Our office in South Miami is roughly 230 miles from Four Corners, about three and a half hours by car, most of it on Florida's Turnpike. Because testing and the pre-surgery diet can happen near home, the trip is kept for the steps that need Dr. K. Arrange for another adult to drive you home after surgery.
For many patients who have lost weight on diets and regained it, surgery is the first tool that changes hunger and portion size at the same time. It does the most good alongside the eating plan, activity and follow-up visits that come with it. Dr. K will tell you honestly at your consultation whether surgery, a nonsurgical procedure or medical weight loss fits your health and goals.
There is no single number that decides it. Eligibility depends on your BMI together with weight-related conditions such as diabetes, high blood pressure or sleep apnea. Patients who carry less excess weight may qualify for Dr. K's Low BMI Mini Sleeve, and he reviews your history before recommending any procedure.
It varies with your starting weight, how closely you follow the post-surgery diet and how active you are. Weight usually comes off fastest in the first months, then slows through the first year. Dr. K walks through a realistic pace for your situation at your consultation and tracks it at every follow-up.
Weight loss injections must be stopped at least two weeks before your operation. Many patients come to us after a plateau on GLP-1 medication, and Dr. K can discuss whether to use it again later as part of your long-term plan.
An upper endoscopy performed within the last five months is accepted. If you need a new one, you can have it near home or with Dr. Keshavarzi, in which case the procedure is included in your overall cost and a $750 hospital fee applies.
Tell us about your weight history and goals. Dr. K will recommend the option that fits, and our team will map out which steps happen near home and which happen with us.
