Dr. Reza Keshavarzi reads an endoscopy of your pouch and outlet before he recommends anything, and he performs bypass, sleeve and endoscopic procedures himself, so the plan fits what changed rather than the one operation on offer.
A gastric bypass revision is a second procedure, or a medical plan, that restores the restriction a Roux-en-Y bypass has lost over time.
Your bypass created a small stomach pouch joined to the small intestine by a narrow connection. Years later the pouch can hold more, the connection can relax and hunger can come back. Each change has a different fix, so the revision is chosen only after we know which one happened to you.
Dr. K performs primary gastric bypass, gastric sleeve and endoscopic sleeve gastroplasty, and he oversees our medical weight loss program. That range lets him recommend the route your findings support, including no surgery at all.
Pompano Beach patients are cared for at our South Miami office by the same surgeon from the first visit to long-term follow-up.
Most people who ask about a revision did well after their bypass. These are the patterns Dr. K asks about when you meet.
A real share of the weight you lost has returned, slowly, over several years.
Portions that once stopped you now leave room, and hunger returns soon after meals.
Your loss stopped well short of the goal you and your first surgeon set.
Blood sugar, blood pressure or joint pain that improved after surgery is heading back up.
Not every case needs an operation. If your pouch and outlet look as they were built, a GLP-1 medication with a structured diet may be the better first step, and Dr. K will say so plainly.
Book a ConsultationSurgeons revise a bypass in several ways. Your endoscopy, your eating pattern and your health goals decide which one fits.
| Route | What changes | Approach | Often considered when |
|---|---|---|---|
| Endoscopic outlet tightening | Stitches placed from inside narrow a loosened connection | Through the mouth, no incisions | The outlet has widened but the pouch is close to its original size |
| Surgical pouch or outlet revision | The pouch is reduced or the connection rebuilt smaller | Laparoscopic, through small incisions | The pouch has clearly stretched |
| Limb length adjustment | More intestine is bypassed, so fewer calories are absorbed | Laparoscopic, through small incisions | Regain continues while pouch and outlet look sound |
| GLP-1 medication and diet plan | A weekly medication quiets appetite alongside your bypass | No procedure | Regain is moderate, or as support before and after a procedure |
At your consultation Dr. K explains the route he recommends for your findings and why the others rank lower for you. Many patients end up with a combination, such as a procedure followed by medication support.

MD, FACS, FASMBS. Board-certified bariatric surgeon
Dr. K is our lead surgeon and conducts every consultation himself. He brings decades of experience in bariatric and minimally invasive surgery to each revision plan.
He trained in surgery at Jackson Memorial Hospital, then completed a fellowship in laparoscopic, bariatric and robotic surgery at Jackson South Medical Center. Revision patients benefit from that depth because one surgeon can weigh endoscopic, surgical and medical answers side by side.
Our team supports you through every preparation step, and your surgery date moves as quickly as your testing does.
Dr. K goes through when and how your bypass was done, your weight since, how you eat now and any health conditions that have returned. You leave knowing the route he recommends and what it requires.
An EGD shows your pouch and outlet as they are today, and medical clearances confirm you are ready. Dr. Keshavarzi can perform the endoscopy, or a recent one from another facility can be used.
A surgical revision is preceded by a required pre-surgery diet. Weight loss injections stop at least 2 weeks before your date.
Dr. K performs the route agreed at your consultation, endoscopic or laparoscopic.
Our surgical patients typically return to work 4 to 7 days after their procedure and avoid lifting more than 20 lbs at first. Dr. K confirms the timeline for your specific revision.
You rebuild meals with the staged diet plans on our website, and Dr. K keeps seeing you as you progress.
Revisions are priced to the route you need and the testing you have already finished, so you receive your figure after the evaluation rather than from a general list.

Cherry is accepted for surgery, and our team walks you through financing options at your consultation.

It is worth it when the evaluation finds a specific, fixable cause, such as a stretched pouch or a widened outlet, and you are ready to rebuild your eating habits around it. When your anatomy still looks as it was built, medication and a structured diet often do more than a second operation. Dr. K tells you which group you fall into before you commit to anything.
Your price depends on the route Dr. K recommends, whether endoscopic, laparoscopic or medical, and on the testing you have already completed. You receive an exact figure after your evaluation. Financing through Cherry offers 24 months at 0% interest with monthly payments as low as $320.
You can request a consultation with Dr. K directly through this page. At that visit he reviews your original bypass and your weight history since, then explains whether a revision, medication or a combination makes sense for you.
Yes. An upper endoscopy, or EGD, shows the size of your pouch and outlet today, which decides the route. Dr. Keshavarzi can perform it, in which case it is included in your procedure cost and a $750 hospital fee applies. An endoscopy done elsewhere within the last 5 months is also accepted.
Weight loss injections must be stopped at least 2 weeks before surgery. If a GLP-1 is part of your plan after the revision, Dr. K sets when to restart it. Many patients use the medication as a bridge while they prepare.
Most patients drive down for the consultation, the procedure and key follow-up visits. Required tests can be done at our clinic or at an outside facility closer to home, and results from our recommended doctor come back within 48 hours. Surgery dates often open within the current month once testing is complete.
